HHCI registered nurses coordinate all aspects of care. From
the first visit to routine follow-up visits, nurses educate the
patient, and family members about medication, home medical
equipment, and self-administration techniques. The hours of
nursing care may include short term, intermittent, extended
care, home visits and 24 hour care.

Case Management
Case Managers plan, coordinate, and monitor a
wide range of services that meet the needs of
older adults with very little family support. We
begin with a comprehensive assessment of
health, psychosocial, environmental, and social
service needs. We develop an individualized
package of services, and resources which
includes a 24 hour caregiver service.
Medical Social Workers
Medical Social Workers assist families to understand and
explain the statement of patient rights. They meet emotional
needs by assisting with acceptance of losses, changes in
family relationships, financial status, and dealing with stress.
